To create a new work design survey, login to your FlourishDx admin dashboard. Using the menu on the left of the screen, navigate to 'Surveys', then 'Work Design Survey'. Click the blue 'Create New Survey' button in the top right corner of the screen. This will open a new screen with four steps to create your new survey.
Step 1 - Enter a survey title
The survey title will automatically populate with "Work Design Survey [Month] [Year]". You can keep this survey title, or you can create your own title. Your title should be worded so that it can be distinguished from other surveys as they populate over time.
If you are running one work design survey across the whole organisation, the default title may be sufficient. However, if different teams are running work design surveys independently of each other, the title should easily distinguish between teams.
A useful format might be "[Team name] Work Design Survey [Month] [Year]". Once you have entered your survey name, click the blue 'Next' button.
Step 2 - Choose Respondents
The next page will display all of the users in your organisation or, for line managers the users to whom you have permission to send surveys. To select all users, simply check the box at the top left of the table. Otherwise, you can apply filters or use the 'Search Users' function to select particular respondents.
You must choose at least eight respondents if you want to view survey results.
The automated dashboard will only populate once eight responses have been received, so ensure your surveys have at least eight respondents if you want to use the analytics capabilities of FlourishDx.
Once you have selected your respondents, click the 'Next' button.
3 - Customise Survey
This screen displays tabs for each of the three domains associated with the integrated model of workplace mental health - Prevent Harm, Promote Flourishing, and Mitigate Illness. A fourth tab provides a set of Outcome measures.
- The Prevent Harm domain includes scales that explore work factors known to contribute to employee stress and psychological ill-health.
- The Promote Flourishing domain includes scales that explore work factors known to contribute to employee flourishing and positive mental health.
- The Mitigate Illness domain includes scales that assess the presence of symptoms of psychological ill-health.
- The Outcomes tab includes a number of outcome measures that are likely to be influenced by each of the three domains.
You can select factors from all four tabs, or you may choose to focus on one area of the integrated model, such as Promote Flourishing, and select factors from that domain alone.
Select the question-mark icon next to a factor to view its definition and the questions that will be included in your survey when that factor is selected.

To select all of the factors within a domain, check the box next to "Factor" at the top of the list. Otherwise, check the box next to the name of each factor you want to include in your survey. Factors can be deselected by unchecking the box.
The 'Total Questions' and 'Time Estimate' sections at the top of the page will update each time you select or deselect a factor. The question mark icon next to this section provides guidance on the length of your survey.
Once you are satisfied with your selection of factors and outcomes, click the 'Next' button.
4 - Review and Send
This screen provides a preview of the email that will be sent to all respondents. When you are happy to proceed, click the 'Create' button. This will send an email notification to all respondents, as shown in the preview.
