To create a new work factors risk assessment survey, login to your FlourishDx admin dashboard. Using the menu on the left of the screen, navigate to 'Surveys', then 'Risk Assessment'. Click the blue 'Create New Survey' button in the top right corner of the screen. This will open a new screen with four steps to create your new survey.
Step 1 - Enter a survey title and include check-in
The survey title will automatically populate with "Risk Assessment [Month] [Year]". You can keep this survey title, or you can create your own title. Your title should be worded so that it can be distinguished from other surveys as they populate over time.
If you are running one risk assessment survey across the whole organisation, the default title may be sufficient. However, if different teams are running risk assessment surveys independently of each other, the title should easily distinguish between teams.
A useful format might be "[Team name] Risk Assessment [Month] [Year]".
You can also add a Check-In to the start of your risk assessment.
Once you have entered your survey name and decided whether to include a check-in, click the blue 'Next' button.
Step 2 - Choose Survey Type
FlourishDx provides two methods of administration - via user accounts or via external link.
The 'User Accounts' option will allow you to target FlourishDx account holders. Users will receive a notification of the survey request. It restricts the survey to one response per account. It can save time as users do not need to enter profile information already on file.
The 'External Link' option will generate a unique URL to be circulated to respondents using your own communication channels. There is no way to track completion or restrict who completes the survey. Users will be asked demographic information at the beginning of the survey for data segregation later. This option can be useful for deskless workforces, or where respondents do not have a company-affiliated email address.
Step 2a - (User Accounts) Choose Respondents
When you choose a User Accounts survey, the next page will display all of the users in your organisation or, for line managers, the users to whom you have permission to send surveys. To select all users, simply check the box at the top left of the table. Otherwise, you can apply filters or use the 'Search Users' function to select particular respondents.
You must choose at least eight respondents if you want to view survey results.
The automated dashboard will only populate once eight responses have been received, so ensure your surveys have at least eight respondents if you want to use the analytics capabilities of FlourishDx.
Once you have selected your respondents, click the 'Next' button.
Step 2b - (External Link) Choose Demographics
When you choose an External Link survey, the next page will show the list of demographic fields available to include in your survey. Tick the box next to a field name to include it in your survey.
3 - Customise Survey
Choose Work Factors
This screen displays the full list of work factors, also known as psychosocial hazards, available for inclusion in your risk assessment. To view the definition of a work factor, click the question mark next to the work factor name.

To include a work factor in your risk assessment, check the box to the top right of the work factor. To include all of the work factors in your risk assessment, check the 'Select All' box. Work factors can be de-selected by clicking on the checked box to remove the tick icon.
Once you have finished your selection, navigate to the 'Promote Flourishing' or 'Outcomes' section.
Promote Flourishing Measures
The Promote Flourishing screen provides the option to include questions about the positive aspects of work within your survey. The items on this screen are all de-selected, so if you do not wish to include outcome measures in your risk assessment, you can skip over this tab.
To view the definition and question set for the Promote Flourishing scales, click the question mark next to the scale name.
Once you have finished your selection, navigate to the 'Outcomes' section.
Choose Outcome Measures
This screen displays the full list of outcome measures available for your risk assessment. To view the definition of an outcome measure, click the question mark next to the outcome name. This will provide a definition of the outcome and the associated questions.
To include an outcome measure in your risk assessment, check the box next to the outcome name. To include all of the outcomes, check the box next to 'Outcome'. Outcomes can be de-selected by clicking on the checked box to remove the tick icon.
If you do not wish to include outcome measures in your risk assessment, de-select all boxes.
Once you have finished your selection, click the 'Next' button.
4 - Review and Send
This screen provides a preview of the email that will be sent to all respondents. When you are happy to proceed, click the 'Create' button. This will send an email notification to all respondents, as shown in the preview.
